Athletes need to turn in the following paperwork:
- Acknowledgment and Consent Form
- Athlete’s Health Insurance – Photocopy of Insurance Card
- Student Emergency Card
- Sports Physical
Parents planning on driving students to athletic contests: please complete the volunteer packet on the district website, as well as the transportation form.
